You will be at the heart of supporting our brilliant customers who have complex lifestyles to achieve positive outcomes. Concentrating on our customers goals and aspirations we provide person centred support, and we are flexible in our approach. This can be anything from attending appointments, court, probation, drug and alcohol services, maximising benefits, claims and supporting with day to day living skills. Tenancy sustainment is also important, and we offer support to enable our customers to achieve this. We empower our customers to develop the independent living skills they need, to successfully move on. About you It is really important that you are; Passionate and able to support customers to live their best life Experienced in creating new, person-centred support plans and supporting others working to existing support plans Experienced in coordinating and assessing customer referrals Experienced working on your own initiative, remaining clam under pressure and having a resilient approach Experienced and have worked in a supported housing service or similar environment before To make sure that we keep our records straight, and customers safe, it's really important that you are technologically savvy (or at the very least not a technophobe!) You ll be given the latest IT kit, which could include a Surface Pro tablet and smart phone, supported by our new online integrated IT system. This means that you don t have to be in the office as you can complete support plans on the go. As this is a community-based job and due to our customer s needs, you need to be able to drive, as well as having access to a vehicle insured for business purposes. Don t worry, we ll pay your expenses to cover this use! When you join as a Support Coordinator, you ll need to have a valid Enhanced Check under the Disclosure and Barring Service. The great news is that we will pay for that to be done if you don t have a transferrable one. Working hours You ll work Monday to Friday 9am to 5pm. Although we do need you to be flexible to meet the needs of our customers so this could include some later evenings up to 8pm, but we aim to give you the time you need to spend doing the things that matter to you!
