We are looking for an experienced Team Manager to join Adult's MASH Team
Location: Greater Manchester
Pay rate will be GBP40/Hr.
Your key responsibilities will include:
Social Care leads will work across all 5 cluster areas to support the delivery of core and extended primary care, ensuring statutory duties and responsibilities of social care are adhered to and embedded in accordance with the legislative framework for an Adult Service user group and their carers.The social care lead will work in partnership with agencies and professional disciplines to identify and deliver opportunities to address health and social care inequalities. In addition being instrumental in supporting the Cluster lead to improve the quality of services including improved outcomes and enhanced service user experience for residents within each cluster area.Leads will ensure staff are supported to deliver the statutory duties and requirements as part of an integrated team. Ensuring the service and its staff are flexible, reliable and responsive to the social care needs of Service Users, Carers and Families.In addition the leads they will ensure the delivery of high quality, effective and efficient health care activities across professional disciplines which support the delivery of integrated pathways and address health and social care inequalities. Including' embedding pathways which support the Greater Manchester Transforming Care agenda. Ensuring the service and its staff are flexible, reliable and responsive to the care needs of Service Users, Carers and Families. Essential Requirements:
