Locum Band 6 Rapid Response Physiotherapist – West London

We have a fantastic opportunity for a locum Physiotherapist to join a leading teaching hospital, based in West London. They are seeking a qualified Physiotherapist with previous Rapid Response experience.
Job Description
The role is for an Advanced Assessment Therapist whose responsibility include completing community–based patient assessment with the aim to avoid unnecessary hospital admission. The candidate is expected to have a valid driving licence with access to a vehicle for work purposes. The role will also involve supporting acute Trusts with discharge facilitation for service users referred via the D2A pathway. During assessments, the clinician is expected to be able to assess and identify the need for social care packages, medication concordance review as well assess for any equipment required to enable the service user to remain safely in their home environment. The candidate may have to carry some of the equipment in their vehicle and is expected to utilise clinical expert knowledge and skills to identify a deteriorating patient then work on an escalation plan accordingly. Successful candidate is expected to be a team player, good advocate for the service users, be able to complete comprehensive documentation and feedback to MDT as well as liaising with other agencies involved in patient care.
Position Requirements
HCPC Registered3 month (minimum) locum assignment to start as soon as possibleFull time hours (Monday – Friday)The successful candidate will be working within a strong team.Minimum of two years postgraduate experiencePrevious clinical experience performing a similar role within a Rapid Response setting.
